Any one knows a Good FORM Script/Builder?

Hello Friends,
Any one knows a good FORM Builder, expect the one in the Fantastico.
I am sure many of the webmasters have FORMS in there site.
What is the Best FORM Scripts you use ? Advice please..

which builds the form, and user fills it, and then they click Submit, and then automatically system sends them email with Word Document, which includes all the info they filled online,
and asking them to sign the word document ?

I would say the best way to do this is to actually lern how to do it yourself, that way you know what's in the script and you can rely on it. With a generated script without the knowlage you could get all sorts of exploits. There are a wast knowlage base on the internet of how to do this in various script languages. I know that this is probebly not what you want to hear but it's an option.
